1. What This Page Is For
This page allows California residents and residents of other states with comparable privacy laws to exercise their rights to:
- Opt out of the “sale” of personal information.
- Opt out of the “sharing” of personal information for cross-context behavioral advertising.
- Limit the use and disclosure of sensitive personal information to purposes necessary to provide the Services.
- Opt out of targeted advertising.
When you submit a request below, we will stop sharing your personal information with advertising partners for targeted advertising and stop passing your information to paying partner providers, to the extent required by applicable law.
2. How We Use Your Information
Urgent Care Online collects information from consumers who express interest in connecting with a licensed telehealth provider. As described in our Privacy Policy, we may share this information with independently licensed providers in our network who compensate us for connecting them with prospective patients, and with advertising partners for purposes of targeted advertising. These activities may constitute “sale” or “sharing” under California and other state privacy laws.
3. Submit a Request
To opt out or to exercise any of the rights described above, submit a request using one of the following methods:
Option A: Email
Email us at [email protected] with the subject line “Do Not Sell or Share My Personal Information” or “Limit Use of Sensitive Personal Information,” and include:

- Your state of residence
- The specific right you are exercising
Option B: Request Form
[A web-based request form will be available at this location. In the interim, please use the email method above or the contact information below.]
Option C: Global Privacy Control
We honor Global Privacy Control (GPC) browser signals as valid opt-out-of-sale and opt-out-of-sharing requests for California residents and other state residents whose laws recognize GPC. To enable GPC, install a compatible browser or browser extension such as those listed at globalprivacycontrol.org.
Option D: Authorized Agent
You may designate an authorized agent to make a request on your behalf. Your authorized agent must submit written authorization signed by you and proof of the agent’s identity. We may ask you to verify your own identity directly.
4. Verification
Before acting on your request, we will take reasonable steps to verify your identity. The verification process may involve confirming information you have previously provided to us. We will not use information provided for verification for any purpose other than verifying your identity and responding to your request.

We will acknowledge your request within ten (10) business days and respond to your verified request within forty-five (45) days. If we need additional time, we will notify you in writing and may extend the response period by up to an additional forty-five (45) days.
6. Non-Discrimination
