SMS Program Terms

Program name: Urgent Care Online Alerts. Program sponsor: Optimal Health Network LLC d/b/a Urgent Care Online, 16092 Coastal Highway, Lewes, Delaware 19958.

1. Program Description

When you submit a form on urgent-care-online.com or a partner form that displays our offer and check the consent box, you agree to receive recurring automated text messages (SMS/MMS) from Urgent Care Online and the partners named in the disclosure at the mobile number you provided. Messages may include confirmation of your request, information needed to connect you with a provider, appointment coordination, reminders, and marketing messages about healthcare provider matching, urgent care, insurance, and related health and wellness offers.

2. Message Frequency and Cost

Message frequency varies based on your request and interaction. Message and data rates may apply. Check with your wireless carrier for details about your plan. Carriers are not liable for delayed or undelivered messages.

Consent to receive text messages is not a condition of purchasing any product or service. You may request a connection without consenting to texts by emailing care@urgent-care-online.com. By consenting, you confirm you are the account holder or have the account holder's permission for the number provided and that you are at least 18 years old.

4. How to Opt Out

Reply STOP to any message to cancel. You may also reply QUIT, END, CANCEL, UNSUBSCRIBE, or REVOKE. After you text STOP, you will receive one final message confirming that you have been unsubscribed. You can also opt out through our Do Not Call / Do Not Text form or by emailing donotcall@urgent-care-online.com. Opt-out requests are honored within ten business days or sooner as required by law.

6. Supported Carriers

Messages are supported on major U.S. carriers, including AT&T, T-Mobile, Verizon, and others. Delivery is subject to carrier availability and is not guaranteed.

7. Privacy

Mobile information will not be shared with third parties or affiliates for their own marketing or promotional purposes without your consent. Mobile information is used and shared only as described in our Privacy Policy, including with partners named in the consent disclosure you accepted and with service providers that deliver messages on our behalf.

We may update these terms at any time by posting the revised version on this page.
